Summa Pride Clinic - Part-time LISW
Summary of Position:
Demonstrates expertise in clinical social work, working as a member of an inter-disciplinary team comprised of providers, nurses and social worker. Occasionally plans educational offerings and ensures continuing education requirements are met. Applies specialized knowledge of the LGBTQ+ community and behavior and social, economic, and cultural systems in directly assisting patients and families to improve or restore their capacity for social functioning including counseling and use of psychosocial interventions and social psychotherapy. Performs adult gender affirming surgery evaluations, psychotherapy.
Minimum Qualifications:
- Formal Education Required:
- Master’s Degree in Social Work or Related Field
- Current Ohio LISW License
- Experience & Training Preferred:
- One (1) to three (3) years of clinical social work experience
- At least (1) year clinical experience with the LGBTQ+ population, preferred
- Demonstrated competency in
theconducting diagnostic assessment, and ongoing therapy with the young adult and adult outpatient population. - Completion of The Gender Affirmative and Supportive Surgery Evaluation Tool (Gender ASSET) within 1 month of employment
- EMR experience, specifically EPIC
- Possesses sound knowledge of community resources/agencies that serve the LGBTQ+ population
- Ability to work well in team environment by supporting team goals and encouraging other team members while being self-directed in executing responsibilities within a team structure.
- Ability to take the initiative to contribute beyond the scope of formal duties (i.e., suggesting innovative ideas and providing feedback on a team’s direction.)
- Ability to effectively interact with diverse populations of patients/customers from a variety of ethnic and socioeconomic backgrounds and an understanding of their needs for self-respect and dignity.
- Ability to communicate verbally and through documentation while maintaining patient confidentiality.
- Ability to react calmly and effectively in emergency situations; ability to work under pressure and time constraints.
- Ability to maintain objectivity in regard to patients/family unit, the department, hospital and community.

Essential Functions:

Direct Patient Care
- Encourages patients to “Do ask, do tell” your healthcare provider so we place the patient at the center of care.
- Delivers social work services to individuals, families and groups through the development of a psycho-social treatment plan; assesses problem areas of patients to include housing, insurance, transportation, mental health and family support.
- Performs assessments of patients who are referred to the center, including but not limited to support of transgender and nonbianary clients preparing to undergo surgery as a part of their gender transition.
- Assumes responsibility for on-going case management, including supportive counseling and crisis intervention.
- Keeps documentation current and accessible to appropriate medical personnel and maintains department records.
- Identifies and accesses resources to meet patient and family needs.
- Participates and collaborates with social service agencies designed to address the multiple needs of the LGBTQ+ population.
- Develops and maintains positive working relationships with external community agencies as well as inter- hospital groups and departments
- Provides consultative support to staff, including nurses, social workers, employees, and providers, as well as community service providers to improve the care of LGBTQ+ patients in the community.
- Supports and participates in professional and community education programs.
- Educates patients and their families related to resources and other needs.
- Participates in the System Behavioral Health Integrated Care Learning Community meetings on a regular basis as well as other meetings identified by behavioral health.
- Participates in SHMG Pride Clinic meetings as identified.
- Helps to bridge the gaps in service and facilitate continuity of care to the LGBTQ+ population and to enhance communication between service providers across the continuum.
- Delivers social work services adhering to the Social Work Code of Ethics.
- Maintains current social work licensure by attending continuing education offerings.
- Educates team members to social work principals of self-determination, autonomy and the medical, psycho-social model of care.
